(a) To be acceptable for registration:
  (1) the name of the person at the time of the birth and the date and place of birth entered on a delayed registration of birth shall be supported by at least:
    (A) two documents, only one of which may be an affidavit of personal knowledge, if the birth occurred at least four years but less than 15 years before the date of the application; or
    (B) three documents, only one of which may be an affidavit of personal knowledge, if the birth occurred 15 or more years before the date of the application; and
  (2) at least one piece of acceptable documentary evidence shall establish to the satisfaction of the State Registrar the name of at least one parent.
(b) The State Registrar shall determine the acceptability of all documentary evidence submitted.
  (1) Documents must be from independent sources and shall be in the form of the original record or a duly certified copy thereof or a signed statement from the custodian of the record or document.
  (2) Documents may include but are not limited to:
    (A) census records;
    (B) hospital records;
    (C) military records;
    (D) Social Security records;
    (E) school records; or
    (F) other documents as designated by the State Registrar.
  (3) For persons 15 years of age or older, all documents submitted in evidence, other than an affidavit of personal knowledge, must be at least five years old.
  (4) At least one document submitted in evidence should have been created within the first 10 years of life.
  (5) Documents shall not contain contradictory information regarding the person's name, date of birth, and place of birth. The identity of at least one parent must be established by information that does not contradict any other information available to the State Registrar. If documents contain contradictory information regarding a second parent, and the delayed certificate of birth is accepted for registration, then no information for the second parent will be recorded on the certificate.
