The organization and physical makeup of the disclosure statement or any revisions thereof shall follow the format and instructions contained in CCRC Form #6 (Format for Disclosure Statement) and CCRC Form #6a (Instructions for Preparation of CCRC Disclosure Statement for Filing with the Texas Department of Insurance).
