(a) The provider shall keep all books and records in a form and manner to accurately reflect the condition of the provider. The books and records shall contain all of the facts essential to its effective operation.
(b) The provider shall provide the department free access to all books and records relating to the business operation of the facility.
(c) The provider must keep the department advised at all times of the physical location of the books and records relating to the business operation of the facility.
