The provider must submit a completed CCRC Form #3 (Officers and Directors page) and CCRC Form #4 (Biographical Data Form) or CCRC Form #4 a (Biographical Data Form for Not-for-Profit CCRC Board Members) to the department within 60 days of any additions, deletions, or modifications to the provider's officers, directors, trustees, managing partner, general partner, any person who has at least 10% interest in the provider, or any person engaged in the day to day management of the facility, other than an individual directly employed by the provider.
