To maintain eligibility for program benefits, the recipient must continue to reside in Texas, be in financial need as defined by this subchapter, and inform the provider in writing or by telephone within 30 days of changes in the following:
  (1) permanent home address;
  (2) health insurance coverage;
  (3) employment;
  (4) other income; or
  (5) family composition.
