Upon termination of any agent's employment by an automobile club, the club shall within 30 days thereafter notify the secretary of state of such termination. Such notification shall be made on the prescribed termination notice form. The form or specifications pertaining to the prescribed form may be obtained by writing to the Statutory Documents Section, Office of the Secretary of State, P.O. Box 12887, Austin, Texas 78711.
